Case Study : Patient With Frozen Shoulder On The Right Upper Arm

Ali, a 40-year-old male, came to the hospital with a case of Frozen Shoulder on the right upper arm. There was severe restriction of upward elevation of shoulder joints. Both active and passive (movement done by the therapist) movements of upper limbs are restricted. Pain is constantly manifesting, and it becomes worst at night and when the weather is colder in Birmingham. Mr. Ali is unable to perform even small tasks due to restricted upward movement of limbs. There is always a felling of heaviness on the affected shoulder.

So What Is Frozen Shoulder?

The medical name of frozen shoulder is adhesive capsulitis. Frozen shoulder is caused dues to long term immobilisation of the arm. Frozen shoulder occurs when the capsule consisting of bones, tendons and ligaments thickens and tightens around the shoulder joint, restricting its movement. This can be because of:

  • Surgeries
  • Fractures or
  • Stroke

Also, Diabetes mellitus and inhaling smoke can be a risk factor for frozen shoulder. Common tasks which are hindered when one suffers from Frozen shoulder are:

  • overhead working like hanging clothes
  • lifting weights
  • combing your hair
  • scratching your back with the affected arm or hand
  • fastening one’s own belt

Frozen shoulder can be correlated with a disease described as Tahajjure wa salabat e kataf in Unani classical. Tahajjure wa salabat e kataf is a humoral (extracellular fluids such as secreted antibodies, complement proteins, and certain antimicrobial peptides) disease in which good humor is replaced by bad humor causing frozen the entire joint structure, especially capsules. Therefore, the treatment and management of frozen shoulder with the help of regimental therapy (massage and cupping) was important.

When Ali came with stiffness, pain and unable to move arm beyond a limited range of motion, the therapist started assessment and evaluation of him. History revealed he had a road traffic accident where his right shoulder had a fracture. Doctors had advised casting the area and immobilisation for 6 weeks.
